In case of withdrawal, the fee(s) less a $150 service charge will be refunded if written notice of withdrawal is received by the National Board by the following dates:

Examination fees will be forfeited if a candidate does not take the examination or if the written notice of withdrawal is received by the National Board after the above dates.

The ($25) State Law Fee, the ($400) Late Registration Fee and the ($300) Special Site Fee are non-refundable fees. Each of these fees must be paid at time of application.

Notification of withdrawal must be in writing and emailed to nbeo@optometry.org. The request must include:

  • Name
  • SSN (last 4-digits only)
  • Examination
  • Current site from which you are withdrawing
  • A note that you are requesting to be withdrawn from the examination
