In order to qualify for licensure, an applicant must submit the following information: Application fees payable to the Indiana Professional Licensing Agency. The fees may be paid together or separately. Application fee: $200.
Official transcripts sent directly from an accredited optometry school, certifying professional degree.
Official score report sent directly from the National Board of Examiners in Optometry showing passing scores in all parts, including the Treatment and Management of Ocular Disease examination.
One (1) passport-quality photograph taken not earlier than one (1) year prior to filing an application, dated and signed on the back, "I certify that this is a true photograph of me." An applicant is required to attain a score of 75 or above on a written jurisprudence examination. The examination will cover the following statutes and rules: Optometry Statute (IC 25-24)
Health Professions Bureau Standards of Practice (IC 25-1-9)
Optometry Rules (Title 852)
Optometric Legend Drug Prescription Advisory Committee Statute (IC 25-25-15)
